Medical Records:
Information about your medical care at the Huebert Sports and Spine Center becomes part of your medical record, which is protected by strict confidentiality laws. If you would like us to release information about your health care, we will ask you to sign a form authorizing us to do so. We will then be happy to transfer that information to anyone you choose.

Referrals:
If you have you been referred by another doctor to the Huebert Sports and Spine Center please notify the receptionist. Some insurance companies require a referral from your primary care physician before you can see a specialist for your condition. If you have this type of insurance plan, you will need the referral form at the time of your visit.

Prescription Refills:
Your doctor must authorize the pharmacist to refill your prescription. It's a good idea to call the pharmacy a couple of days before you need the prescription. Prescriptions can be refilled only during our regular business hours. To request a prescription refill please call our office at (503) 885.8008 and choose the prescription refill option.
